To help make your first visit to our practice as smooth as possible, please read the information on this web page. And, you can save some time by completing the new patient forms in advance of your appointment. We utilize an online records intake service, known as Phreesia, which allows patients to complete patient forms online prior to arriving for their appointments. If you are a new patient to our practice, you will receive a link that is sent via text (SMS) to your cell phone (smart phone) three days prior to your appointment. We ask that you please complete these forms prior to arriving for your appointment.
